Here at Boulevard Pharmaceutical Compounding Center, we are dedicated to helping you feel better. Our pharmacists can evaluate your specific hormone-related symptoms and formulate a therapy tailored to you.
Step 1: Confidential Evaluation
As part of a comprehensive approach, Bio-identical Hormone Replacement Therapy (BHRT) analyzes a Men’s Hormone Evaluation form. The patient questionnaire inquiries about your health history as well as the symptoms you are experiencing. Patient questionnaire answers help us to identify the symptoms that bother you the most. Step 2: Test results from hormone labs
In conjunction with the Hormone questionnaire, we also require the following labs to be completed:
- Testosterone
- Luteinizing hormone (LH)
- Follicle-stimulating Hormone (FSH)
- Prolactin
These tests can be ordered by your physician and then the results are forwarded to us.
Step 3: Consultation
After receiving both the Hormone questionnaire and your hormone test results, one of our pharmacists will reach out to you to schedule a consultation. During your consultation, the evaluation and hormone levels will be reviewed, as well as any other concerns you may have. In addition, specific nutritional interventions may be addressed at that time.
Step 4: Recommendation
After your consultation, our pharmacist will consult with your provider and give our recommendations for your bio-identical hormone therapy.
